Annual health review

If you received a text message asking you to visit this page, it is because you are due an annual health review.

Each year we invite a large percentage of our patients for an annual long-term illness review. This is for a range of conditions, including, but not limited to:

  • Cardiovascular/Heart Disease including Atrial Fibrillation, Heart Failure and High Blood Pressure (Hypertension)
  • Chronic Kidney Disease (CKD)
  • Diabetes
  • Stroke and Transient Ischaemic Attacks (TIAs)
  • Peripheral Arterial Disease

An Annual Review is required for all patients with a chronic disease. The purpose of an annual review is to ensure patients are taking the right medication and receiving the best possible care for their condition(s).

We encourage our patients to take an active role in helping to manage their own conditions, but it is helpful to attend the practice at least once a year for a review.

The majority of our annual reviews are carried out by our nursing team.

Please use the link to book your 15 minutes appointment with our practice nurse or HCA.

 

During your review, the Practice Nurse or HCA will;

  • Take a non fasting bloods sample checking: U&E, LFT, Cholesterol, and Hb1ac
  • Check your blood pressure and pulse
  • Check your height and weight
  • May take a urine sample

 

Your review appointments can be booked; 

Please note review appointments are bookable between 08:30 -13:00 Mondays to Fridays.

Please make sure you drink plenty of liquid before your blood test.

It is important to attend your review appointment so we can ensure your condition is properly controlled and managed. If you run out of your repeat medication and have not had a review, we may not be able to re-authorise your repeat medication.
